Fundraiser Requests

Please forward any fundraiser requests to Ann Petit at petita@mishawaka.k12.in.us

Fundraisers must be approved by the building Principal and the Central Office prior to the start of the fundraiser. A signed copy will be emailed back to the requesting sponsor and the school office.

Please email or call Ann at ex. 4500 with any questions or concerns you may have regarding fundraisers.

Fundraiser Follow-Up Report
This report may be completed by the sponsor at the conclusion of the fundraiser activity and given to the ECA Treasurer.

Fundraiser Request Form
Use this form for all fundraiser requests. Return the form to the Superintendent's Office.

Fundraiser Sign-Out Form
This form must be used whenever students are given items to sell. A completed copy of this form must accompany the SA-8 when turning in money to the ECA Treasurer.

Recipient Listing of Incentives or Awards
This form must be completed and sent to the Business Office whenever gift cards are given out.
